Before your visit, please complete our new patient forms. You may also fill them out in the office. We ask that you arrive 15 minutes early so we can review them with you.

If you have dental insurance, please bring your insurance card or completed insurance form so we can keep a copy on file. Once we have your information in our system, you only need to update us if there's a change to your employer or coverage.
To assist with your care, we also ask for the contact information for your previous dentist so we can request your most recent x-rays in advance of your appointment.

Yes, we are happy to provide a free insurance verification. Email a photo of the front and back of your insurance card to ritam@drfayhu.com, along with your name, date of birth, and the subscriber's name and date of birth. We will contact your provider and send you a written summary of your coverage, usually within 1-2 business days.
Yes. If you'd like an exact estimate of what your insurance will cover, we can submit a pre-treatment estimate on your behalf. This process requires your address and your insurance ID or the subscriber’s ID (which may be a Social Security Number). Please allow 2–4 weeks for the estimate to be processed and returned to our office.
We are an out-of-network provider. However, if you have a PPO insurance plan with out-of-network benefits, you will likely still be able to apply your benefits toward your care with us.
Although we are out-of-network, many of our patients find the level of care we provide to be well worth it. We do not accept Medicaid or HMO plans. However, some patients with Medicare or Medicaid may carry PPO dental plans such as Cigna or United Health Dual Complete, which we may be able to work with.
